People who don't buy health insurance do not pose a "free-rider" problem. In fact, the extra taxes they pay undoubtedly cover the cost of "uncompensated" care. I have written an article explaining this (http://tinyurl.com/5h4yvc).
Very few of the chronically ill suffer "cosmic injustice". Some do, but much chronic illness is due to lifestyle choice. If you have community rating, how do you reward people for living healthy? Shouldn't they pay lower premiums?
